Why Is This Role Critical?

The Care Team Pod is a multidisciplinary team of licensed and unlicensed staff who provide direct support and care to Upward Health’s patients. The Care Team Pod is comprised of a diverse team that may include registered nurses, licensed social workers, pharmacists, therapists and paramedics, community health workers, peer support specialists, and addiction counselors. This team works within the community and in the patients’ homes or meet in agreed upon locations in the community. 


The Addiction Counselors are trained to provide support and assistance to our patients living with substance use disorders (SUD).  This role offers a range of direct services to patients that include addiction counseling support, collaborative goal setting, care coordination across all medical and behavioral providers, and emotional support. 



  1. Support the Patients’ Recovery Goals        
  • Develop care plan and guides with patient and providers regarding the patient’s health management goals
  • Engage patients to achieve health management goals using recovery coaching, motivational interviewing, and problem-solving techniques
  • Coach and assist patients in overcoming any barriers to meeting recovery goals and update care plans accordingly
  • Assist and accompany patients in scheduling appointments and accessing community resources
  • Arrange for member transportation to recovery and wellness services appointments
  • Accompany patients as indicated to recovery and wellness services appointments


Follow up with patients via phone calls, home visits, or other locations meeting patients where they are

  • Work with patients one-on-one and in groups to provide recovery training and outreach to individuals who use mental health services in the community
  • As appropriate, share personal recovery experiences and develop authentic peer-to-peer relationships
  • Keep accurate and timely documentation in accordance with company policies


  1. Support Community Integration of Participant


  • Work with facilities to help transition patients after discharge to a safe home environment
  • Address all social determinants of health by working with social service agencies to meet other the patient’s needs (housing, food, clothing, financial assistance, etc.)
  • Proactively follow up with individual to ensure he or she is making a successful transition to community integration and is continuing their progress toward recovery goals
  • Continuously expand knowledge of community resources, services, and programs available to participants and build ongoing relationships with these organizations to advocate for patients


  1. Improve Organizational Service Delivery
  • Keep treatment team informed about individual’s strengths, accomplishments, and obstacles in relation to their recovery goals – accomplished through daily huddles, interdisciplinary team (IDT) meetings, one-to-ones, etc.
  • Participate in focus groups, hiring committees, work groups, etc. to help shape policies and procedures that benefit SUD program participants
  • Perform other duties as assigned




  • Counseling skills, including evidenced-based practice model knowledge, clinical assessment, and timely case documentation
  • Professional presentation of self and representation of company
  • Interpersonal savvy, with the demonstrated by the ability to interact with and influence people to establish trust and build strong relationships
  • A high sense of urgency and can-do attitude required for a role at a start-up company
  • Ability to do home visits and outreach
  • Comfortable with computer data entry
  • Strong organization skills and ability to manage and maintain a personal schedule
  • Ability to establish priorities and meet deadlines
  • Ability to work independently within a virtual operating environment and as part of a team
  • Excellent oral and written communication skills
  • Ability to carry out written and oral instructions
  • Ability to exercise judgment in the application of professional services
  • Customer service proficiency
  • Liaison skills with legal and other agencies




  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with Bachelor’s degree, or Licensed Masters-level degree in Social Work or Mental Health Counseling, with CASAC preferred
  • Spanish language proficiency preferred
  • Two years’ experience in addiction counseling preferred
  • Availability Monday through Friday and must include evenings
  • Ability to exhibit a nonjudgmental approach, effective listening, good eye contact, and positive interactions
  • Long-time resident of the community with good knowledge of the resources of this community
  • A valid driver’s license and auto liability insurance
  • Experience with or strong interest in working in health, social, education, or community services
  • Knowledge of community resources in applicable geographic area
  • Experience working as a community health workers or peer support specialist is a plus
  • Able to maintain clear professional boundaries with patients and coworkers
  • Working knowledge of social and health issues
  • Ability to complete Upward Health’s initial training program and ongoing educational requirements as assigned
  • Commitment to represent the company with professionalism
  • Cultural competency – able to work with diverse groups of community patients
  • Basic computer skills, including ability to type




  • Multi-lingual capabilities preferred, but not required
  • Strong communication and problem-solving skills
  • Technologically savvy is a plus
  • Prior experience as an Addiction Counselor
